Workshop Participation & Waiver of Liability Agreement
Healthy Once and For All™ Program Terms & Conditions
Effective Date: Upon Enrollment
The Healthy Once and For All™ Program ("Program") is an eight (8) week coaching, accountability, education, and support program provided by Healthy Mindset Rebels LLC ("HMR").
By enrolling in the Program, making payment, accessing Program materials, attending Program sessions, or otherwise participating in the Program, you ("Customer") agree to be legally bound by these Terms and Conditions.
1. Payment
Payment Options
Option 1 – Pay in Full
The full Program tuition is due at the time of enrollment.
Option 2 – Two-Payment Plan
50% of Program tuition is due at enrollment.
The remaining 50% is due during Week 4 of the Program.
By selecting a payment plan, you authorize HMR to charge the payment method on file for scheduled payments.
Failure to remain current on payments may result in suspension or termination of Program access.
2. Transfer, Default, Cancellation & Termination
Transfer Policy
You may transfer your enrollment one (1) time to:
A future Program date
Another Healthy Mindset Rebels Program
Another individual
The first transfer is free.
Any additional transfer requests will incur a $150 transfer fee.
Transfer requests must be submitted via email to:

Default
Your account must remain current throughout your participation in the Program.
Cancellation
You may cancel without penalty only where required by applicable law and within any legally mandated cancellation period.

Refund Processing
Any refund required by applicable law will be issued to the original payment method used during enrollment.
Missed Sessions & No-Show Policy
No refund, credit, extension, makeup session, or additional coaching will be provided for missed sessions.
If you miss a live session, HMR may provide access to a replay.
You are solely responsible for reviewing and completing all missed content within 24 hours.
Teaching Weeks
Weeks 1, 3, 5, and 7 are designated teaching weeks.
If you miss any of these sessions, you are required to catch up within 24 hours using:
Program replays
Program portal materials
Approved YouTube content provided by HMR
Program resources and worksheets
Failure to review missed content does not create entitlement to:
Refunds
Credits
Extensions
Makeup sessions
Additional coaching
No-Refund Policy
Enrollment reserves a limited Program space and requires HMR to commit coaching resources, support services, Program materials, and community access.
Except where prohibited by law:
All sales are final.
No refunds, credits, chargebacks, partial refunds, or prorated refunds will be provided for:
Failure to attend
Failure to participate
Scheduling conflicts
Personal circumstances
Dissatisfaction with results
Voluntary withdrawal
Removal for violation of these Terms
3. General Provisions
Entire Agreement
These Terms constitute the entire agreement between you and HMR.
Governing Law
These Terms are governed by the laws of the State of Arizona.
Program Changes
HMR reserves the right to modify:
Program schedules
Content
Delivery methods
Instructors
Program structure
at any time.
Termination
HMR may suspend or terminate participation for:
Violations of these Terms
Harassment
Disruptive conduct
Abuse of staff or participants
Community guideline violations
Non-payment
Assignment
You may not assign or transfer this Agreement without written consent from HMR.
Additional Releases
HMR may require additional waivers, acknowledgements, releases, or policies as a condition of participation.
Intellectual Property & Recording Restrictions
You may not:
Record Program sessions
Share Program materials
Screenshot proprietary content
Republish Program materials
Share login credentials
Upload Program content to public websites or social media
All Program materials remain the exclusive intellectual property of Healthy Mindset Rebels LLC.
Media Release & Consent
You grant HMR an irrevocable, perpetual, worldwide, royalty-free right to use your:
Name
Image
Photographs
Video
Audio recordings
Testimonials
Success stories
Transformation stories
Before-and-after photographs
Statements
Likeness
for lawful business purposes.
This may include use in:
Websites
Social media
Marketing campaigns
Advertising
Funnels
Landing pages
Sales pages
Email campaigns
Webinars
Presentations
Educational materials
Printed materials
Testimonials
Case studies
Future products and services
You waive any right to compensation, approval, inspection, or review of such materials.
Testimonials and results do not guarantee future outcomes.
This consent survives termination of the Program.

The Program is educational and informational in nature.
HMR does not provide:
Medical advice
Diagnosis
Treatment
Healthcare services
You should consult qualified healthcare professionals before implementing any health, nutrition, exercise, supplement, or lifestyle changes.
Results Disclaimer
Individual results vary.
HMR makes no guarantees regarding:
Weight loss
Health improvements
Lifestyle outcomes
Personal transformation
Any specific result
Limitation of Liability
To the fullest extent permitted by law, HMR shall not be liable for any direct, indirect, incidental, consequential, special, exemplary, or punitive damages arising from participation in the Program.
Assumption of Risk & Release of Liability
You voluntarily assume all risks associated with participation in the Program, including but not limited to:
Nutritional changes
Exercise recommendations
Lifestyle modifications
Behavioral changes
Emotional coaching
You agree to release and hold harmless HMR, its owners, employees, contractors, coaches, affiliates, agents, successors, and representatives from claims arising from participation in the Program to the fullest extent permitted by law.
Arbitration & Dispute Resolution
Any dispute arising from these Terms shall first be addressed through good-faith discussions.
If unresolved, disputes shall be resolved through binding arbitration in Arizona.
You waive the right to a jury trial.
Client Participation Requirements
You agree to:
Attend Program calls and meetings whenever reasonably possible.
Arrive prepared with requested worksheets, accountability materials, numbers, and tracking information.
Review missed session replays within 24 hours.
Catch up on all missed content from Weeks 1, 3, 5, and 7 within 24 hours.
Remain current with Program lessons and assignments.
Participate actively in the Healthy Mindset Rebels community, including WhatsApp and other designated platforms.
Utilize DC Formula Sheets and Program tools provided by HMR.
Conduct yourself respectfully and professionally.
Refrain from harassment, bullying, discrimination, abusive conduct, or disruptive behaviour.
Failure to participate fully may negatively impact results and does not entitle you to refunds, credits, extensions, or additional coaching.
HMR reserves the right to suspend or terminate participation without refund for violations of these Terms or conduct harmful to the Program community.
Confidentiality & Community Privacy
You agree to maintain the confidentiality of information shared by other participants.
You may not:
Share
Publish
Screenshot
Record
Distribute
Disclose
participant information without permission.
Violation may result in removal from the Program without refund.
WhatsApp & Community Guidelines
You agree to:
Treat others respectfully
Avoid harassment or abusive conduct
Refrain from soliciting or selling to other participants
Respect participant privacy
Follow all community guidelines established by HMR
HMR may remove participants from community platforms or the Program for violations.
Electronic Consent & Signatures
Electronic acceptance, enrollment, payment, checkboxes, digital signatures, and participation constitute acceptance of these Terms.
Electronic signatures shall have the same legal effect as handwritten signatures.
Chargebacks & Payment Disputes
You agree to contact HMR and attempt good-faith resolution before initiating a chargeback or payment dispute.
HMR may use:
Enrollment records
Payment records
Attendance records
Communications
Login records
Participation records
to defend against chargebacks or payment disputes.
